Important Filling Instructions
To ensure your application is not rejected, follow these mandatory guidelines provided in the form:

- Photographs: Affix two recent colour photographs (size 4.5 cm x 3.5 cm) with a white background.
- Signature Placement: You must sign or provide a left thumb impression across the photo on the left side and within the box provided on the last page.
